Abstract

A method of predicting future blood glucose concentrations of an individual patient includes: selecting an individualized nonlinear physiological model of glucose-insulin dynamics, the selected model having a plurality of model parameters whose values are to be determined; estimating values for each of the model parameters in the plurality of model parameters, a first subset of the model parameters having values estimated from a priori population data and a second subset of the model parameters having values personalized for the individual patient by applying a parameter estimation technique to a priori information and data for the individual patient to obtain a posteriori information; and; applying a nonlinear prediction technique to the selected model using the estimated values for each of the model parameters to obtain a predicted blood glucose concentration of the individual patient at a future time.

Description

INCORPORATION BY REFERENCE TO RELATED APPLICATION

[0001] Any and all priority claims identified in the Application Data Sheet, or any correction thereto, are hereby incorporated by reference under 37 CFR 1.57. BACKGROUND

[0002] Type 1 diabetes (T1D) is a chronical autoimmune disease caused by the progressive destruction of beta cells in the pancreas, which leads to the inability of producing endogenous insulin by the organism. As a result, blood glucose (BG) concentration tends to exceed the hyperglycemic threshold (BG>180 mg/dl), a situation that, if frequent and prolonged, could lead to several serious cardiovascular long-term complications, as well as nephropathy and neuropathy. To reduce BG levels, administration of exogenous insulin several times a day is necessary. Unfortunately, excessive exogenous insulin dosing could lead patients to hypoglycemia, i.e., BG<70 mg/dl, which is dangerous even in the short-term since it could cause fainting, light-headiness, coma and even death.

[0003] Effective T1D treatment relies on BG frequent monitoring, made through either the classic fingerstick device or more modern minimally invasive continuous glucose monitoring (CGM) sensors, and is far from being trivial. Indeed, T1D management represents, from a patient perspective, a life-long learning process to understand how several everyday factors (e.g., illness, diet, and physical activity) affect BG levels and how interventions (e.g., rescue carbohydrate intake and, of course, insulin administration) can be used to keep BG in the safe range.

[0004] In this context, many efforts have been made by the research community to provide new tools able to help patients with T1D. Among them, CGM-based algorithms able to predict future BG concentration in real-time have the potential to significantly improve T1D therapy efficacy by enabling proactive therapeutic decisions based on the expected future glucose levels, rather than the current one.

SUMMARY

[0005] In a first aspect, a method is provided of predicting future blood glucose concentrations of an individual patient, comprising: selecting an individualized nonlinear physiological model of glucose-insulin dynamics, the selected model having a plurality of model parameters whose values are to be determined; estimating values for each of the model parameters in the plurality of model parameters, a first subset of the model parameters having values estimated from a priori population data and a second subset of the model parameters having values personalized for the individual patient by applying a parameter estimation technique to a priori information and data for the individual patient to obtain a posteriori information; and; applying a nonlinear prediction technique to the selected model using the estimated values for each of the model parameters to obtain a predicted blood glucose concentration of the individual patient at a future time.

[0006] In an embodiment of the first aspect, the parameter estimation technique is a Markov Chain Monte Carlo (MCMC) Bayesian estimator.

DETAILED DESCRIPTION

I. Introduction

[0028] Despite several different methodologies having been proposed for blood glucose (BG) real-time prediction, the problem remains open due to several unknown disturbances altering BG levels and to the high inter-/intra-patient variability in glucose physiology. The existing prediction techniques rely on a mathematical model which describes the relationship between a certain set of input features and BG. The choice of a suitable model of glucose-insulin regulation is an important step and the options spans from “black-box” models, completely data-driven, to models based on mechanistic/semi-mechanistic nonlinear description of metabolic physiology. Compared to black-box models, physiological models have the clear advantage of guaranteeing to correctly capture the underneath glucose-insulin metabolism mechanisms. Among the nonlinear physiological models available in the T1D literature, the so-called minimal models prove too rigid and simplistic to grant effective glucose prediction. On the other hand, large-scale models have several equations and multiple parameters which make them hardly identifiable and more suited to computer simulations than to model-based prediction.

[0029] FIG. 1 is flowchart illustrating at a high level the steps involved in the individualized model-based prediction scheme described herein. As shown, the first step 10 in the scheme selects a suitable model structure from among the various available options described above. The selected model is an individualized nonlinear physiological model of glucose-insulin regulation. As described below, the selected model has model parameters that can be individualized from easily accessible patient logs, such as carbohydrate intake and insulin infusion data, to mimic an individual specific physiology.

[0030] In the second step 20 of the prediction scheme shown in FIG. 1 selected ones of the model parameters that are to be personalized for an individual patient are estimated using a parameter estimation technique. In one embodiment the parameter estimation technique that is used employs a Bayesian approach, implemented by Markov Chain Monte Carlo (MCMC) to estimate the large number of parameters in the presence of complex nonlinear dynamics. Next, in the third step 30 the obtained personalized model is used within a nonlinear prediction scheme based, for example, on a particle filter methodology.

[0031] As will be further described in more detail below, the prediction scheme described herein is tested using an in silico clinical trial comprising data of 100 virtual T1D patients generated by a simulation model described in R. Visentin, E. Campos-Nuñez, M. Schiavon, D. L v, M. Vettoretti, M. Breton, B. P. Kovatchev, C. Dalla Man, and C. Cobelli, “The uva/padova type 1 diabetes simulator goes from single meal to single day,” J. Diabetes Sci. Technol., vol. 12, no. 2, pp. 273-281, 2018, which is hereby incorporated by reference in its entirety. For comparison, two baseline black-box models are considered. The metrics chosen for the assessment are the root mean square error, the coefficient of determination, and the time gain. Results show that the proposed approach is superior than both baseline methodologies in predicting future glucose concentration especially in terms of time gain.

II. A Physiological Model of Glucose-Insulin Regulation

[0032] This section describes one example of a nonlinear physiological model of glucose-insulin dynamics for personalized glucose prediction. This example starts from the maximal physiological model implemented in the most recent version of the UVa/Padova T1D Simulator (T1DS) described in the Visentin et al. reference cited above. and simplifies it as much as possible to reduce the number of parameters to be estimated for individualization while retaining its ability to achieve good glucose prediction.

[0033] The proposed model has two inputs, insulin infusion I(t), and carbohydrate intake CHO(t), and one output, the interstitial glucose concentration IG(t). The model is composed of three subsystems describing subcutaneous insulin absorption, oral glucose absorption, and glucose-insulin kinetics.

A. Subcutaneous Insulin Absorption Subsystem

[0034] The subcutaneous insulin absorption model is a slightly simplified version of the one used in the T1DS model, described in M. Schiavon, C. Dalla Man, and C. Cobelli, “Modeling subcutaneous absorption of fast-acting insulin in type 1 diabetes,” IEEE Trans. Biomed. Eng., vol. 65, no. 9, pp. 2079-2086, 2018 and illustrated in FIG. 2. The model is composed of three compartments. Exogenous insulin I is infused to the first compartment where it appears after a delay β. In the first compartment, representing insulin in a non-monomeric state, insulin is transformed in a monomeric state and then diffused to the plasma. The model equations are:

where I_sc1(mU/kg) and I_sc2(mU/kg) represent the insulin in a non-monomeric and monomeric state, respectively; I_p(mU/l) is the plasma insulin concentration; k_d(min⁻¹) is the rate constant of diffusion from the first to the second compartment; k_a2(min⁻¹) is the rate constant of subcutaneous insulin absorption from the second compartment to the plasma; k_e(min⁻¹) is the fractional clearance rate; V_l(l/kg) is the volume of insulin distribution; β (min) is the delay in the appearance of insulin in the first compartment. The simplification with respect to the model used in the T1DS consists in neglecting the fact that a fraction of non-monomeric insulin can reach the plasma directly. This absorption route is depicted in gray in FIG. 1 and associated to the rate constant k_a1. The simplification is due to the fact that both the insulin fraction and the number of patients where this happens is small. A priori information on model parameters, reported in Table 1, have been obtained from the literature. Specifically, V_Iand β have been set to population values, i.e. 0.126 l/kg and 8 min, respectively. Furthermore, k_dhas been constrained to k_d≥k_a2since the two combinations are interchangeable. Unknown model parameters result θ_ins=[k_a2, k_d].
B. Oral Glucose Absorption Subsystem

[0036] The oral glucose absorption subsystem model, taken from C. Dalla Man, M. Camilleri, and C. Cobelli, “A system model of oral glucose absorption: Validation on gold standard data,” IEEE Trans. Biomed. Eng., vol. 53, no. 12, pp. 2472-2478, 2006, represents a simplified version of the model used in the T1DS and is illustrated in FIG. 3. It describes the gastro-intestinal tract as three-compartment system: the first two compartments account for food in the stomach (solid and grinded state), while the third compartment models the upper small intestine where CHO is absorbed. where Q_sto1(mg/kg) and Q_sto2(mg/kg) are the glucose amount in the stomach in a solid and liquid state, respectively; Q_gut(mg/kg) is the glucose concentration in the intestine; k_gri(min⁻¹) is the rate constant of grinding; k_empt(min⁻¹) is the rate constant of gastric emptying; k_abs(min⁻¹) is the rate constant of intestinal absorption; CHO (mg/kg/min) is the ingested carbohydrate rate. [0038] The simplification with respect to the model used in the T1DS consists in assuming a constant gastric emptying rate, thus neglecting its dependence on stomach content (depicted in gray in FIG. 3).

[0039] A priori information on model (2) has been obtained from the literature and has been detailed in Table 1. In particular, we set f equal to 0.9 and we constrained k_gri=k_empt. Furthermore, k_abshas been constrained to k_abs≤k_emptsince the two combinations are interchangeable. As such, the unknown model parameters are θ_oral=[k_abs, k_empt].

C. Glucose-Insulin Kinetics Subsystem

[0040] With respect to the model used in the T1DS simulator, we used a more parsimonious description of the glucose-insulin kinetics subsystem. It is based on a well-known two-compartment model that describes the impact of the plasmatic insulin action and glucose rate of appearance in plasma glucose concentration introduced in R. N. Bergman, C. R. Bowden, and C. Cobelli, “Quantitative estimation of insulin sensitivity,” Am. J. Physiol., vol. 236, no. 6, pp. e667-e677, 1979. The model is further equipped with a third compartment to describe the transport of glucose from plasma to the interstitium where it is measured by the sensor. The model is illustrated in FIG. 4. Model equations are:

III. Offline Bayesian Estimation of Personalized Model Parameters by Markov Chain Monte Carlo

A. Implementation Details

V. Realtime Glucose Prediction Through Particle Filter

[0071] Up to this point, we focused on estimating the parameters of the physiological model of a T1D subject, to capture the patient-specific dynamics. This process can be done “offline” using the available information obtained from retrospective patient data.

[0072] In this section, we will present how we used such a personalized model to predict, in real-time, future glucose concentrations. This task has to be performed “online”, so we resorted to a sequential algorithm that at each timestep t_k, when a new measurement y(t_k)=CGM(t_k) becomes available, updates the current estimate of the model state x(t_k) and uses it to infer future glucose concentration. In particular, we employ the particle filter (PF), which is a sequential Bayesian prediction technique capable of handling the nonlinear structure of the model.

VI. Methodology Assessment

A. The Simulated Dataset

[0102] T1DS is a software tool that has been accepted by the FDA as a substitute to pre-clinical trials for closed-loop insulin delivery strategies and open-loop therapy. It provides a population of 100 virtual adult subjects, each characterized by a different vector of physiological parameters to capture the inter-/intra-subject variability of T1D population. In this particular example described herein, we used the most updated version of the simulator, capable to better account for inter-/intra-patient variability (i.e., changes over time in a patient's physiology, and patient behaviour. Using the T1DS, two weeks of synthetic data have been generated for 100 virtual adults, simulating the so-called Sensor Augmented Pump therapy (SAP). In this therapy regimen insulin is administered to the patient by an insulin pump as the sum of two components: the first component is the basal insulin infusion rate, a fixed daily pattern adjusted to each patient by the clinician; the second component consists of multiple insulin doses administered at each meal to counterbalance the corresponding glucose increase. The amount of insulin injected is proportional to the estimated carbohydrate intake (CHO) and pre-prandial glucose measurement.

[0103] In our simulated scenario, each subject ate 3 meals per day taking place with uniform probability in the intervals [06:30, 08:30] (breakfast), [12:00, 14:00] (lunch), and [19:30, 21:30] (dinner) with CHO consumption uniformly distributed in [20, 60], [50, 90], and [50, 90] g, respectively. A simple safety protocol, consisting in consuming 15 g of fast-acting CHO each 15 minutes while the patient's measured glucose is below 70 mg/dl, was implemented to treat hypoglycemia. Finally, the measurement error of the CGM sensor was modeled as proposed in A. Facchinetti, S. Del Favero, G. Sparacino, and C. Cobelli, “Model of glucose sensor error components: Identification and assessment for new dexcom g4 generation devices,” Med. Biol. Eng. Comput., vol. 53, no. 12, pp. 1259-1269, 2015. The obtained in silico dataset has been split into training and test set by assigning the first week of data to the former and the second week of data to the latter.

B. Prediction Approaches Used for Comparison

[0104] As a baseline for prediction accuracy, we considered with two state-of-the-art black-box models: we trained on the same training data an Auto Regressive with eXogenous input (ARX) model of order 5, and an Auto Regressive Integrated Moving Average with eXogenous input (ARIMAX) model of order (5,5,1) both having two inputs, insulin infusion I(t) and carbohydrate intake CHO(t), and one output, the glucose concentration CGM.

C. Assessment Metrics

[0105] Evaluation of prediction accuracy has been performed in the test set by comparing the obtained glucose predictions with the actual CGM values using different PH. In this work, three metrics have been considered: the root mean square error (RMSE), the coefficient of determination (COD), and the time gain (TG). D. Results

[0111] FIGS. 6A and 6B show RMSE and COD obtained with ARX, ARIMAX, and PF for different PH, from 15 min up to 120 min. The tick line represents the median over the population, while the area between the 25th and 75th percentile is shaded. The same information is reported in Table II for PH=30, 60, 90 and 120 min, that includes also the p-values.

[0112] Compared to ARX, PF achieves significantly better outcomes (p<0.05) for all considered metrics and PH, the higher PH the bigger the improvement. In detail, the average ΔRMSE=RMSE_PF−RMSE_ARXis −1.47 mg/dl (−6%), −3.85 mg/dl (−15%), −7.83 mg/dl (−23%), and −11.78 mg/dl (−28%) for PH=30, 60, 90, and 120 min, respectively. The same considerations hold when the prediction accuracy is evaluated in terms of average ΔCOD which improved by 2.19% (2%), 9.86% (14%), 20.30% (40%), and 32.99% (118%) for PH=30, 60, 90, and 120 min, respectively. Focusing on ΔTG, PF remarkably improves the outcomes by 15.0 min (100%), 40.0 min (200%), 67.5 min (300%), 95.0 min (380%) for PH=30, 60, 90, and 120 min, respectively.

VII. Alternative Embodiments

[0115] The individualized model-based prediction scheme presented herein in connection with the flowchart of FIG. 1 has been illustrated using exemplary examples of a particular individualized nonlinear physiological model (a maximal model such as the UVa/Padova T1D Simulator), a particular parameter estimation technique (MCMC) and a particular nonlinear prediction scheme (a particle filter methodology). More generally, the individualized model-based prediction scheme may employ a variety of alternative parameter estimation techniques and a variety of alternative nonlinear prediction schemes may be used in combination with different individualized nonlinear physiological models. For example, in some embodiments the parameter estimation technique that is employed may be a maximum likelihood technique or a prediction error minimization method. Likewise, in some embodiments the nonlinear prediction scheme that is employed may be an extended Kalman filter technique or an unscented Kalman filter technique.

VIII. Conclusion

[0116] Personalized real-time prediction of glucose concentration in T1D is important to enable effective proactive therapeutics. We have presented a methodology that uses a personalized physiological model to achieve this goal. In some embodiments this gray-box nonlinear model is handled in a Bayesian framework comprising two main phases. In the first phase, we use MCMC, for example, to identify patient-specific parameters of a physiological model of a T1D subject. Then, in the second phase, the personalized model is used by a particle filter or other nonlinear prediction scheme to infer future glucose data. The first phase may conducted offline, retrospectively processing patients historical data. The second phase may run online as new patient data becomes available.

Claims

The invention claimed is:

1. A method of predicting future blood glucose concentrations of an individual patient having a continuous glucose monitoring (CGM) sensor, comprising:

selecting, by a processor, an individualized nonlinear physiological model of glucose-insulin dynamics, the selected individualized nonlinear physiological model having a plurality of model parameters whose values are to be determined;

estimating, by the processor, population-based values of a first subset of the plurality of model parameters from a priori population data, wherein the estimated population-based values of the first subset relate to at least one of insulin absorption or glucose absorption;

estimating, by the processor, personalized values of a second subset of the model parameters by applying a parameter estimation technique to a priori information about a probability distribution of each parameter of the second subset and data for the individual patient, the data for the individual patient comprising insulin infusion information for the individual patient and carbohydrate intake information for the individual patient;

obtaining, by the processor, an a posteriori probability function based on past glucose measurements of the individual patient from the CGM sensor, the estimated population-based values of the first subset of the plurality of model parameters, and the estimated personalized values of the second subset of the plurality of model parameters;

receiving, by the processor, a current glucose measurement of the individual patient from the CGM sensor;

updating, by the processor, the posteriori probability function based on the current glucose measurement;

applying, by the processor, a particle filter to the selected individualized nonlinear physiological model using the updated a posteriori probability function to obtain a predicted blood glucose concentration of the individual patient at a future time;

generating, by the processor, a notification based on the predicted blood glucose concentration indicating upcoming dysglycemia, the notification comprising a proactive alert of the upcoming dysglycemia; and

causing, by the processor, the notification to be output on a device associated with the individual patient.

2. The method of claim 1, wherein the parameter estimation technique is a Markov Chain Monte Carlo (MCMC) Bayesian estimator.

3. The method of claim 1, wherein the parameter estimation technique is selected from the group consisting of a maximum a posteriori technique, a maximum likelihood technique and a prediction error minimization technique.

4. The method of claim 1, wherein the selected individualized nonlinear physiological model includes a subcutaneous insulin absorption sub-model, an oral glucose absorption sub-model and a glucose-insulin kinetics model.

5. The method of claim 4, wherein the subcutaneous insulin absorption sub-model includes a first model parameter specifying a value of insulin distribution and a second model parameter specifying a delay in an appearance of insulin in a first compartment, the first and second model parameters being in the first subset of the plurality of model parameters having the estimated population-based values.

6. The method of claim 5, wherein the subcutaneous insulin absorption sub-model includes a third model parameter specifying a diffusion rate constant from a first to a second compartment and a fourth model parameter specifying a rate constant of subcutaneous insulin absorption from the second compartment to plasma, the third and fourth model parameters being in the second subset of the plurality of model parameters having the estimated personalized values.

7. The method of claim 1, wherein the selected individualized nonlinear physiological model is a maximal physiological model in which selected ones of the plurality of model parameters are neglected.

8. The method of claim 1, further comprising augmenting the selected individualized nonlinear physiological model with a residual error model to describe residual modeling errors.

9. The method of claim 8, wherein the residual modeling errors are modeled as an Auto regressive Integrated Moving Average (ARIMA) of order (5,5,1).

10. The method of claim 8, wherein the residual error model has model parameters estimated using a predicted error method (PEM).

11. The method of claim 1, wherein the predicted blood glucose concentration is predicted at a plurality of different times.

12. A computer-program product comprising a non-transitory computer-usable medium having computer-readable program code embodied therein, the computer-readable program code adapted to be executed to implement a method of predicting future blood glucose concentrations of an individual patient having a continuous glucose monitoring (CGM) sensor, the method comprising:

selecting an individualized nonlinear physiological model of glucose-insulin dynamics, the selected individualized nonlinear physiological model having a plurality of model parameters whose values are to be determined;

estimating population-based values of a first subset of the plurality of model parameters from a priori population data, wherein the estimated population-based values of the first subset relate to at least one of insulin absorption or glucose absorption;

estimating personalized values of a second subset of the model parameters by applying a parameter estimation technique to a priori information about a probability distribution of each parameter of the second subset and data for the individual patient, the data for the individual patient comprising insulin infusion information for the individual patient and carbohydrate intake information for the individual patient;

obtaining an a posteriori probability function based on past glucose measurements of the individual patient from the CGM sensor, the estimated population-based values of the first subset of the plurality of model parameters, and the estimated personalized values of the second subset of the plurality of model parameters;

receiving a current glucose measurement of the individual patient from the CGM sensor;

updating the posteriori probability function based on the current glucose measurement;

applying a particle filter to the selected individualized nonlinear physiological model using the updated a posteriori probability function to obtain a predicted blood glucose concentration of the individual patient at a future time;

generating a notification based on the predicted blood glucose concentration indicating upcoming dysglycemia, the notification comprising a proactive alert of the upcoming dysglycemia; and

causing the notification to be output on a device associated with the individual patient.

13. The computer-program product of claim 12, wherein the parameter estimation technique is a Markov Chain Monte Carlo (MCMC) Bayesian estimator.

14. The computer-program product of claim 12, wherein the parameter estimation technique is selected from the group consisting of a maximum a posteriori technique, a maximum likelihood technique and a prediction error minimization technique.

15. The computer-program product of claim 12, wherein the selected individualized nonlinear physiological model includes a subcutaneous insulin absorption sub-model, an oral glucose absorption sub-model and a glucose-insulin kinetics model.

16. The computer-program product of claim 15, wherein the subcutaneous insulin absorption sub-model includes a first model parameter specifying a value of insulin distribution and a second model parameter specifying a delay in an appearance of insulin in a first compartment, the first and second model parameters being in the first subset of the plurality of model parameters having the estimated population-based values.

17. The computer-program product of claim 16, wherein the subcutaneous insulin absorption sub-model includes a third model parameter specifying a diffusion rate constant from a first to a second compartment and a fourth model parameter specifying a rate constant of subcutaneous insulin absorption from the second compartment to plasma, the third and fourth model parameters being in the second subset of the plurality of model parameters having the estimated personalized values.

18. The method of claim 1, wherein the notification comprises a recommended treatment to mitigate the upcoming dysglycemia indicated by the predicted blood glucose concentration.

19. The method of claim 1, wherein the device associated with the individual patient comprises the processor.

20. The method of claim 1, wherein the causing comprises communicating, by the processor, the notification to the device associated with the individual patient.
